Marjolein Smidt
Gästföreläsare SFFB kirurgveckan 2022
Prof. dr. Dr. Marjolein Smidt has been working as a surgeon at Maastricht UMC+ since 2008 and is specialized in breast cancer surgery. In 1997 she obtained her medical exam cum laude at the University of Maastricht. After her training, Prof. Dr. Marjolein Smidt further specialized in laparoscopic surgery at the St. Franciscus Gasthuis in Rotterdam. In 2006 she obtained her PhD with her thesis entitled 'The sentinel lymph node in breast cancer patients: an evaluation of new developments in clinical practice'.
Prof. dr. Dr. Smidt is conducting research into axillary gland metastases in collaboration with the Radiology department. In recent years, survival for breast cancer patients has become so good that more attention is being paid to the side effects of the treatment, for example lymphedema as a result of the armpit surgery. If this is possible, no more surgery is required. She is also involved in international cooperation in the field of breast surgery.
During the last decades, breast cancer survival increased impressively as a consequence of extensive adjuvant treatment. Consequently attention shifted from only survival to a combined endpoint including both survival and Qol. This attention shift resulted in the question: “How can we limit overtreatment?”. All research prof. dr. Smidt performs suits this qualification.
Concerning axillary treatment, the ultimate treatment goal is equally safe though with less lifelong morbidity. Therefore she is interested in noninvasive nodal staging study with (PET)MRI, which has the potential to offer the same knowledge as the operative axillary staging without operation. In case of omission of any axillary operation, chances for complications like shoulder stiffness and lymphedema, all significantly impacting quality of life, will drop to almost zero.
Her goals are also supported by the BOOG 2013-08, RISAS- and MINIMAX-study. All these studies are focused on de-escalation of axillary treatment.
· The BOOG 2013-08 RCT investigates the added value of the sentinel lymph node biopsy (SLNB) in clinical node negative breast cancer patients treated with breast conserving therapy. The study just finished the randomizing phase with inclusion of 1730 patients, which might be seen as an impressive achievement.
· The RISAS-study investigated the value of the RISAS-procedure as staging procedure in clinically node positive patients neoadjuvant setting. The RISAS-procedure proved to be the most accurate procedure compared to the marked node and sentinel lymph node biopsy. Thereby it has the highest chance of identification of complete response (when adjuvant regional treatment can be omitted, thereby preventing morbidity and its impact on Qol). At the same time, accurate identification of remaining disease can instigate adjuvant systemic therapy, thereby increasing overall survival. In the Netherlands the RISAS-procedure is the preferred staging procedure now.
Prof. dr. Smidt collaborates with international principal investigators of comparable studies aiming to empower outcome and catalyze implementation.
She is further developing a breast cancer related prevention program. Lifestyle, like alcohol intake and BMI, has proven to highly impact breast cancer incidence as well as recurrence. By providing this knowledge to patients, they have the information to take something in their rollercoaster breast cancer era in their own hands. These patients are supported by dieticians, physiotherapists or lifestyle coaches, depending on the provided breast cancer treatment.
